Lending Integrity Seal of Approval Application Forms

Below are the Lending Integrity Seal of Approval forms that you will need to fill out and send to your states in order to apply to qualify for the Seal.

• Cover Letter for your NAMB State Affiliate (MS Word)
• Continuing Education Verification Form (PDF)
• Letter of Recommendation for New Professional Member (MS Word)
• Letter in Lieu of a Background Check for Loan Officer (MS Word)

Renewing Professional and Loan Originator Members who have had their national background check within the last 7 years can simply sign the following form:
